Carbon County Dog License
Proof of Disability Requirements

To qualify, the person must have any one of the required items listed:
(please contact the Treasurer's office for more information)

• If the person receives Medicare, he/she should supply a copy of their
  social security statement
  
  

• If the person receives disability insurance or supplemental security income
  for the aged, blind or disabled under the Social Security Act.
  (49 Stat. 620, 42 U.S.C. § 301 et seq.);
  
  

• If the person receives who receives a rentaI property tax rebate under the
  act of March 11, 1971 (P. L.1 04, No.3), known as the
  "Senior Citizens Rebate and Assistance Act," on account of disability;
  
  

• If the person has a disability certificate issued by the
  United States Veterans' Administration;
  
  

• If the person's vehicle has a special registration plate / placard
  under 75 Pa.C.S. § 1338.
